Job Summary
This vacancy is being posted at Social Work Senior, Social Work Specialist, and Social Work Specialist Senior levels. The level at which the position will be filled will be determined based on the successful candidate's qualifications.

The Social Work Senior/Social Work Specialist/Social Work Specialist Senior works closely with multi-disciplinary treatment teams, families, court systems, and government and community human service agencies to secure and coordinate mental health services on behalf of patients. Mental health services range from inpatient treatment to placement in less restrictive state operated inpatient or community-based services. Using their knowledge of social work practices, the Social Work Senior/Social Work Specialist/Social Work Specialist Senior provides services including psychosocial assessments, treatment, transfer and discharge services to individuals, groups and families upon client admission and throughout hospitalization.

Primary Responsibilities Include:
  • Providing psychosocial services to patients, groups, families and/or significant others, and assisting and participating in the planning and revision of the ongoing treatment process which includes psycho-social assessments, treatment reviews, and transfer/discharge planning meetings
  • Assessing patient and family needs for information and/or education regarding social services, treatment and continuum of care services to assist in effective use of the treatment program and Social Service Department resources
  • Developing, coordinating and maintaining effective liaison networks with families, county social service agencies and other appropriate community resources to assure continuity of treatment, transfer, discharge and aftercare
  • Participating in departmental quality service and improvement activities, focusing upon enhancement of patient services and social work competency

Minimum Qualifications:

Social Work Senior

__Licensed Social Worker (LSW); OR
__Licensed Independent Social Worker (LISW); OR
__Licensed Graduate Social Worker (LGSW) OR
__ Licensed Independent Clinical Social Worker (LICSW)


__a minimum of 18 months of professional social work experience.

Social Work Specialist

__Licensed Social Worker (LSW)

__a minimum of three years professional social work experience;


__Licensed Independent Social Worker (LISW); OR

__Licensed Graduate Social Worker (LGSW) OR

__ Licensed Independent Clinical Social Worker (LICSW)

__a minimum of 18 months of professional social work experience.

Social Work Specialist Senior

__Licensed to practice in the state of Minnesota at the Independent Clinical Social Worker level
